Please bring your current medical insurance card to every visit and notify me if there is a change in your insurance coverage. 

I ask that patients check their benefits before their first appointment or if their insurance has changed for any reason. If your insurance is billed and it does not cover visits with Naturopathic Physicians, you may be responsible for the cost of the care.
